Campus Recreation & Wellbeing Survey

This study aims to assess student experiences, engagement, and satisfaction with campus recreation programs and facilities at Presbyterian College.
Your participation in this study is voluntary. Please read this form and ask any questions that you may have before agreeing to take part in this study.

Purpose of the Research Study: This survey will look at your experiences with Recreation facilities, activities, and programs and assess your current satisfaction, as well as your needs and expectations. 
Number of Participants: The subjects of this study will be full-time undergraduate students, aged 18 years and older, enrolled at Presbyterian College.
Procedures: Your voluntary participation in this study, which is an online survey consisting of a series of questions about your experience using Campus Recreation facilities, programs, and services. You will be asked relevant demographic questions like age, gender, ethnicity, and year in school. Please be assured that your answers are confidential. No individual’s answers will ever be identified in any report. Not participating will in no way affect your status at the university. 
Length of Participation: It is anticipated that your participation will take approximately 5-10 minutes. You may discontinue your participation at any time, without penalty. 
This study has the following risks: While survey research presents minimal risks, you may be uncomfortable answering some of the questions and can skip questions or withdraw from the survey at any time. 
Benefits of being in the study are: Data collected from this project will be used by administrators to make informed decisions about recreation programming, services, and facilities. 
Confidentiality: The Campus Groups survey is housed on the researcher’s password-protected, duo-security CG account. All electronic information generated as part of this study will be stored in a locked file on the researcher’s password-protected university computer. Data will be kept for three years as per federal guidelines. The survey has been set up so that participation is anonymized; no direct identifiers of any participant will be obtained. Further, you will not be asked anywhere in the survey to share your name or the name of any other individuals, only the name of your university. All data will be reported in the aggregate form and will not be tied to any one individual. 
Compensation: You will not be reimbursed for your time and participation in this study. 
Voluntary Nature of the Study: Participation in this study is voluntary. If you withdraw or decline participation, you will not be penalized or lose benefits or services unrelated to the study. If you decide to participate, you may decline to answer any question and may choose to withdraw at any time.

You understand that participation is voluntary and is not a requirement for receiving benefits or services from Presbyterian College. You understand that you may withdraw from the research study at any time without penalty. 

References to Campus Recreation may include Fitness Centers, Group Exercise, Intramural Sports, Esports, Club Sports, Adventure Trips, Archery, Disc Golf Course, Open Recreation, and Outdoor Fields and Courts.
